Persistent symptoms are a reason to reassess

Treatment-resistant depression commonly refers to depression that has not improved adequately after at least two suitable antidepressant trials. The details matter: medication dose, duration, adherence, side effects, and the diagnosis should be reviewed. The term does not mean that recovery is impossible.

What to bring to a consultation

A useful history lists each medication, how long you took it, the dose if known, what changed, and why it was stopped. Include psychotherapy, augmentation strategies, and any previous TMS or other brain-stimulation treatment.

Consider the whole picture

A clinician may review sleep, physical health, substance use, other medications, bipolar symptoms, and related conditions. Symptoms can persist for different reasons, and the next step should be guided by an evaluation rather than a label alone.

There may be other options

Further medication or psychotherapy approaches, TMS, and other treatments may be considered depending on your situation. At Houston West TMS, Dr. Drell can assess whether Deep TMS is appropriate while coordinating with your current clinician.

Coverage requires a separate review

Insurance requirements may differ from a clinician’s definition of treatment resistance. Your benefits and expected out-of-pocket costs should be clarified before treatment begins.
